ABOUT LA CASA DE DON PEDRO: With a mission to foster self‐sufficiency, empowerment, and neighborhood revitalization, La Casa de Don Pedro is a community based social service and development organization that has been providing comprehensive programming to residents of Greater Newark for the past 40 years. Programs range from early childhood education, youth enrichment, family counseling, adult development, immigration, home energy and weatherization assistance, community building, neighborhood planning and development, housing & foreclosure counseling, to financial counseling and job placement. Its traditional service area is the northern sector of the City of Newark, however, over the last four decades in addressing the needs of the primary service area, the field of work has grown to meet the needs of many residents throughout Newark and the larger metropolitan areas. This is particularly so given the cultural competencies inhabited within La Casa, the growing needs of the community at large, and the growing Hispanic community. On average, 50,000 people are served each year. To address the multiple and complex needs faced by our neighborhoods, La Casa is dedicated to providing a comprehensive network of social services as well as community building programs and services. This is matched by skilled and capable staff and a strong governance structure through its management and board of directors.
PROGRAM SUMMARY: La Casa de Don Pedro welcomes a qualified and passionate community health care coordinator to lead its newest program, the Healthy Living Initiative (HLI). The goal of the HLI is to improve health outcomes for North Newark residents by educating families about health issues, advocating for families by improving their access to health care, health services, and health insurance, and providing individual case management to residents who need extra care to help manage their health situation. The program is predominantly targeted to medically underserved Hispanic adults and children.
PRIMARY RESPONSIBILITIES
The Healthy Living Initiative (HLI) Coordinator will be responsible for implementing the objectives of the HLI program which are as follows:
1. Expand health promotion and access of services within La Casa’s Family Success Centers
2. Improve health literacy and residents’ participation in meeting their own health needs
3. Increase access to health services and health insurance
4. Improve health care and management of individuals with significant medical/care needs.
The HLI Coordinator will
Through research and surveying, identify the most critical health concerns impacting our community;
Educate area residents on the most pressing health concerns by planning and implementing seminars on essential health topics
Identify and follow up with residents who require health interventions
Formalize and/or renew relationships with local health care providers to connect residents to their services
Refer and manage residents to health care and dental providers
Refer residents to available health programs, such as nutrition and fitness education
Accompany residents to doctors’ appointments as necessary, serving as advocate and translator
Home visits to clients who require additional follow up
Evaluate and improve upon results.
Work cooperatively with La Casa’s Family Success Center and Parent‐Child Academy program staff to recruit program participants and follow up with their needs, as well as other La Casa program staff as needed.
Perform other related duties as assigned
EDUCATION AND EXPERIENCE:
Bachelor’s Degree in community health, psychology, or social services highly preferred and three years of community health experience. Associate’s Degree and five years of relevant case management and community health advocacy is acceptable. MPH or Nursing graduates welcome.
JOB REQUIREMENTS AND PHYSICAL DEMANDS:
Must be bilingual English and Spanish
Experience in creative adult education and training
Must have a valid New Jersey Driver’s License – Motor Vehicle Record (MVR) required
Must be proficient with Microsoft Word, Excel and Access
Exceptional communication skills; written and verbal in both English and Spanish
Must be detailed oriented
Experience in building coalitions and partnerships
Ability to work independently as well as part of a team
Must be able to work in a fast‐paced, challenging environment
Must be able to travel for in‐town and out‐of‐town conferences and trainings
SALARY Based on education and experience.
